Initial Consultation for diagnosis and or medication management

80-minute in-person evaluation. Review of required documents requested prior to evaluation. Diagnosis and/or personalized treatment plan tailored to your individual needs. For ages 13 to adulthood.
Executive Function (Life Skills) Coaching

In-depth evaluation with personalized strategies focused on improving executive functioning. MindPrint cognitive testing packages for teens improve success rates and “buy in” from teens. Currently only coaching patients aged 13 and older individually. Parent coaching and adult coaching available.
Medication Management

Medication management is available for ADHD, Anxiety, and Depression. Evaluation required every 3-4 months to monitor medication response and assess the need for adjustments.
MindPrint Cognitive Testing and School Accommodation Recs

MindPrint test completed at the convenience of your home online, and includes 1 hour consultation for results review, and personalized recommendations. 504/IEP recommendations to be shared with school when appropriate available by request for $99.
QB Testing

QbTest is an FDA-approved clinical test for ADHD that aids in diagnosis and treatment. This test can be done with a referral from your PCP, with results sent directly to them, or as an existing client of ADHD Life Medical Associates with a follow-up appointment for interpretation and treatment planning. Ages 6-60 years old.
GeneSight Testing

GeneSight is a genetic, cheek swab test that shows how your genes may affect medication outcomes. It can help prevent medication failures and unwanted side effects, improving overall mental wellness.
